False PHD is a passive Devil Room item added in the The Binding of Isaac: Repentance expansion that flips the mechanics of consumable pills into a high-risk, high-reward damage engine.
Unlike the standard PHD item which turns bad pills into good ones, False PHD forces all pills to become negative or neutral while granting massive combat payouts. Core Effects

The Stat-Down Trade: Converts all positive stat pills in your current run pool into their stat-down counterparts. However, whenever you consume a stat-down pill, you gain a permanent +0.6 Damage Up (+1.2 Damage for massive Horse Pills). This damage bonus applies retroactively for any bad pills you took earlier in the run.
